  3. Submit the completed Carrier Packet and Documents


Step 1: Download the MTA Freight New Carrier Packet

Packet includes:

  • ACH Payment Form

  • Quick Pay Agreement

  • Broker-Carrier Agreement

  • Carrier Profile

  • Payment Instructions

Step 2 - Complete New Carrier Packet

** Insurance Certificate requires specific instructions required to complete application **

Certificate must show as Certificate Holder:

VAJ LLC dba Motor Transport Alliance - 24 S Rock Hill Rd - St. Louis, MO 63119

** Must include coverage for auto liability, cargo liability, and general liability **

Step 3: Submit New Carrier Packet & Documents

At this stage you will need to submit the carrier packet and the documents listed below including W-9 and more.

  • ICC / Operating Authority (FMCSA Certificate or State Permit)

  • Insurance Certificate (Carrier) - Instructions found on page 1 - New Carrier Packet

    • Insurance naming us as
